New Individual Patients
Following are the steps to follow if you have decided you are interested in individual psychotherapy. The initial phone consultation and first appointment are opportunities for you to ask me any questions you have about my services, policies, and approach to therapy, and to share your reasons for seeking therapy so that together we can determine whether I am the best person to help. Often the only way to know that you have chosen the right therapist is to meet and see how it feels in the room.

Contact me by phone or email to ask any questions you may have or to schedule a free 15-minute initial consultation by phone. If you would rather meet in person, contact me to make arrangements for a first appointment.
- Patient Forms
Please print and complete the Patient Information Form prior to your first appointment. Also print and carefully read the document on Informed Consent, which includes information on my office policies, the MN Patient Bill of Rights, and Notice of Privacy Practices (HIPAA). We will discuss this information during our first meeting and sign the document together.
- First Appointment
Please arrive for your first appointment at our agreed upon day and time. Please remember to bring the printed forms and your preferred method of payment. Also be sure to allow enough time to find the office and parking. For transportation and parking information please click here.
Once you arrive, use the entrance at 74 South 9th Street and take the elevator to the 12th floor, suite 1240. Have a seat in the waiting room and I will greet you there. There are keys marked Men and Women in the waiting room that you will need to access the restrooms, which are kept locked for building security. Men’s and women’s restrooms are on alternating floors. The men’s restroom is on the 12th floor. To access the women’s restroom, you will need to take the elevator or stairs either up or down one floor. Please remember to return the key to the waiting room.
